2026-2027 APS Physical Information (Dated after April 1, 2026)
- Physical forms must be completed and submitted via the new DragonFlyMax.com portal, prior to Bear Camp and tryouts. For instruction on how to submit your physical click here
- Physical forms will NOT be accepted by coaches.

- NOTE: Completion of a Concussion Course is required in order to complete the DragonFly process. This is a one hour course.
