APS Athletics participation requirements.

1.  Dragonflymax.com: 

1. In order for students to participate in APS Athletics, all Parents must create an account and complete a questionnaire on www.dragonflymax.com for each of your athletes, every year.. Please note: Only Parents should create an account, DO NOT USE STUDENT INFORMATION, or email, to create an account.

2. One Account Per Family: You only need to create one account per family- You may then add multiple athletes to that account. You will keep this account until you no longer have an APS athlete. You'll be able to merely update your athlete's information on an annual basis.

3. Ensure you select the appropriate sport for each of your athletes and choose La Cueva as your school

4.  Complete all steps and requirements in Dragonfly.com each year.

Physicals:    

• Download and print the physical from Dragonfly.com, or visit the LC Athletics page.
• You will need to upload your athletes' Physical to Dragonfly. 
• Physicals must be dated after April 1 of each year.

 Concussion training certification

• Concussion training certification 
• This is a 1 hour online video course. It will provide a certificate at the end, which You will need to upload to the "Concussions for Students" section on DragonFlymax.com

TECHNICAL DIFFICULTIES???

1. Use a desktop, not a phone or ipad.  
2. If you upload a document and it does not appear to have been uploaded, log out and clear your browsers "cookies" and "cache"
3. When you see that something is in "Pending" status, it means the Physical Trainer has yet to review your account.
